You will need the following information to successfully complete this form:

• Your WA Operator number: Motor vehicles and trailers that operate under a Permit in Western Australia must be registered with Main Roads Western Australia. In order to register vehicles and trailers you must be a registered Operator.
  If you do not have an Operator number, please complete the Restricted Access Vehicle Operator or WA Heavy Vehicle Accreditation Registration form available on our website.
• VIN/Chassis number
• Gross mass (t) (vehicle, combination and trailer where applicable)
• Axle spacings
• Manufacturer's axle group rating (t)
• Aggregate Trailer Mass (ATM) – means the total maximum mass of the trailer, as stated by the manufacturer together with its load and the mass imposed on the towing vehicle by the trailer when the towing vehicle and trailer are on a horizontal surface

A separate form must be completed for each motor vehicle or trailer in your fleet.

2. I hereby declare that I wish to retire from the WA Heavy Vehicle Accreditation Scheme and I am the authorised contact person for the accredited  company / individual specified in this form
3. I understand that retiring from the WA Heavy Vehicle Accreditation Scheme will result in the cancellation of any permits attached to this accreditation, including the permits of any sub-contractor also approved to operate under this accreditation.
4. As the authorised contact person, I agree to receive a verification telephone call from Main Roads to confirm my retirement from Accreditation

Operator Details

A Mass Only Assessment Application can be used to aid the transport industry in planning for upcoming movements on the WA road network by requesting an assessment of the vehicle combination and maximum mass permitted on the specified route.

This application will not result in a permit or approval for travel over the assessed structures.

Vehicle Details

Ground Contact Width (m)

Ground contact width is the distance between the outermost point of the ground contact of the outside tyres on each end of the axle.

Ground contact width in relation to an axle group means the greatest contact width of all the axles in the group.

To apply, the single trip permit nominated must have been issued within the last three (3) months.

The transport operator must have conducted a route survey and confirmed that the route, dimensions, mass or configuration will not be amended following the issue of the copied permit. A new Single Trip permit application will need to be submitted where the above changes are required.
