Incident Reporting
You must complete this form within 48 hours of being involved in an on-road incident involving a RAV operating under a Permit or Order.
You will also need to report crashes to WA Police as required under road law.
Common Incident types
An incident involving a RAV that results in part or all of the combination rolling over.
HVS will work with the transport operator involved to establish the cause of the rollover.
It may be necessary for the transport operator to verify they have appropriate systems in place under their WAHVA to mitigate rollover risks.
A RAV colliding with another vehicle or a stationary object such as a tree, power pole, building or road infrastructure, while travelling on a public road, regardless of who is at fault.
Reporting a crash involving a RAV allows HVS to review the events that led to the incident and to assign appropriate actions as necessary.
A trailer or dolly that has detached from the combination whilst operating on the public road network.
HVS will work with the transport operator involved to establish the cause of the vehicle separation.
It may be necessary for the transport operator to verify they have appropriate maintenance systems in place under their WAHVA.
A near miss involving a RAV means a serious incident almost occurred, but didn't. This could include many things, such as almost hitting cattle on the road, or almost hitting another vehicle turning out of an intersection, or almost rolling over.
HVS will review the events that led to the near miss and assign appropriate actions as necessary.
When a RAV malfunctions causing the vehicle to impact the road network and/or road safety.
This could be an engine failure that has resulted in the vehicle blocking a lane and causing congestion, or it could be brakes seizing on a trailer, or a wheel falling off.
HVS will work with the transport operator involved to establish the cause of the vehicle failure. It may be necessary for the transport operator to verify they have appropriate maintenance systems in place under their WAHVA.
