You will need the following information to successfully complete this form:

  • Your WA Operator number: Motor vehicles and trailers that operate under a Permit in Western Australia must be registered with Main Roads Western Australia. In order to register vehicles and trailers you must be a registered Operator.
    If you do not have an Operator number, please complete the Restricted Access Vehicle Operator or WA Heavy Vehicle Accreditation Registration form available on our website.
  • VIN/Chassis number
  • Gross mass (t) (vehicle, combination and trailer where applicable)
  • Axle spacings
  • Manufacturer's axle group rating (t)
  • Aggregate Trailer Mass (ATM) – means the total maximum mass of the trailer, as stated by the manufacturer together with its load and the mass imposed on the towing vehicle by the trailer when the towing vehicle and trailer are on a horizontal surface

A separate form must be completed for each motor vehicle or trailer in your fleet.
