RAV Operator Form

To operate a Restricted Access Vehicle (RAV) in WA, you must first register as an Operator with Main Roads Western Australia.

You may also require WA Heavy Vehicle Accreditation, depending on the permit type required. 

Please note: You won't be able to save this form mid-way through completion. You may be asked for the following information which we recommend you have prepared before completing the form:

  • Drivers licence
  • Australian Business Number (ABN)
  • Certificate of Registration or Australian Business Register
  • Current Company Extract 
  • Operator number (for request for update to registration details)

The Authorised Contact Person is the only person with the authority to amend RAV registration information. If that person is no longer the Authorised Contact Person, then a new registration must be completed with an authority and declaration signed by a Responsible Person of the organisation. A Responsible Person is an officer of that organisation who is authorised to apply for registration as an operator of a RAV.
If you are not the Authorised Contact for this registration, please provide sufficient evidence of your authority to make these changes (e.g. Letter from Director/Operational Manager).

If you do not have an Australia Business Number, please respond 'Nil'
