Heavy Vehicle Services (HVS) is introducing web forms as part of the transition to provide our customers the ability to do business ‘on-line’ and stop out-of-date application forms being used.

Initially, three web-forms will be introduced on 16 May 2022. These are the:

• Application to List or Update Vehicle Information Form
• Traffic Escort Services Booking Request or Cancellation or Amendment Application Form

The web-forms include the ability to attach supporting documentation to your application if required.

Customers will be given a two-week period, commencing 16 May 2022, to transition to the web-forms, after which any old application forms will be returned and not processed.

A copy of the completed web-form will be provided to customers via the email address nominated when completing the web-form.

HVS will transition the remainder of its application forms to web-forms throughout 2022.
