AMENDMENT TO CANCELLATION & REFUND POLICY – HVO UPDATE 17-2007

Statutory Declaration

Any reference to a director of a company in respect of making a declaration in HVO update
17-2007 shall also include – “a person of authority representing the company”.

Note:

  A Statutory Declaration must be made before an authorised witness.  A list of authorised
  witnesses is attached to the Statutory Declaration form on the link below –

  Cancellation of multiple permits on a single Statutory Declaration is acceptable.

  An example of a person of authority for a company would be the Transport Manager of that
  company.  A person employed as a loader, driver or office assistant would not fall into that
  category.

This amendment comes into effect immediately.
